Worker's Compensation & MVA Referrals

Due to high volumes of worker’s compensation & MVA  invoices remaining unapproved and unpaid by insurance companies as a result of incomplete documentation, our practice operates the following process:
 
Unfortunately for privacy reasons we are unable to invoice your employer.
 
Before we can invoice an insurance company our practice requires these documents:
 
  • Doctor’s written referral
  • Doctor’s First medical certificate or Progress medical certificate indicating allied health or physiotherapy is required (for w/c)
  • Name and address of insurance company where we can send the account (patient’s can ask their employer for this information). 
  • Name of case manager (optional – if known)
  • Claim number (patient is notified of this by the insurance company)
  • Letter of approval from the insurance company (patient is notified directly)
 
Patients are responsible for paying for their own physiotherapy treatment until copies of all the documents have been received at our practice.
 
Once all the required information is received you will be able to claim back any payments that you have made to us during this interim period. Our practice will be happy to invoice any further treatment directly to the insurance company once the above documentation has been received.
